What You Will Do to Change Lives
Under direct supervision, the Case Manager III serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Shifts Available:
Full Time 40 Hours per Week | 8:00am - 4:30pm | Monday - Friday with rotating crisis cell phone during after hours
Expected starting wage range is $21.49 - $26.25. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
What You Bring to the Table (Must Have)
- Bachelor's degree in Social Services or Human Services and (1) year of experience or High School Graduation or G.E.D. equivalent and four (4) years of direct services in Mental Health or community services; an Associate's degree may be substituted for two (2) year of experience.
- Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
- Willingness to use the company vehicle to drive clients to appointments and groups, etc.
- Willingness to use your personal vehicle to attend meetings, etc.

Program Type: Community-based
To Make Referrals:
- ICM: For institutional case management, referrals come directly from the Public Conservator's office and/or coordinated through San Diego's Department of Behavioral Health. This program only serves individuals that are on public conservatorship.
- SBCM: For strength-based case management, referrals come from a variety of places including hospitals, other providers, family members, and individuals. All referrals must be submitted through Optum's Single Point of Access (SPOA)
# of Members: 354 ICM members; 352 SBCM members
Who We Serve: Adult residents of San Diego County, ages 60 and older, who are diagnosed with a serious mental illness. For ICM services, the individual must also be conserved and placed in a locked, residential facility.
Hours of Operation:
- Monday - Friday: 8:00 am- 5:00 pm, evenings and weekends as needed
- Crisis services are available 24/7
